About St. Clair County’s CHNA and CHIP
The information included within this website represent the strategic framework for a data-driven and community-informed Community Health Improvement Plan. Since 2000, by request of the St. Clair County Health Department (SCCHD), a professional consulting company has conducted the St. Clair County Behavioral Risk Factor Survey (BRFS) every five years. This survey provides data for the St. Clair County Community Health Needs Assessment (CHNA). CHNA data collected highlights health status and health behaviors of St. Clair County residents. CHNA identifies key health needs and issues through an organized, thorough data collection and analysis. From the CHNA, a Community Health Improvement Plan (CHIP) will be created and target priority areas to be addressed including involving a wide array of St. Clair County community partners and residents. Over the years, more data that is robust came from secondary data sources.
In 2018, the first CHIP started. In 2021, a comprehensive CHNA was completed, and a new CHIP has been developed with key community partners and will span from 2023-2027. This CHIP highlights several priority areas and identifies strategies for implementation within the community. Together, we are working towards a healthier tomorrow.
